Rachel Smith, CNM, ARNP
Rachel Smith, CNM, ARNP is a graduate of University of Illinois, Trinity College of Nursing and Health Sciences, and Midwifery Institute of Philadelphia University. She has ongoing training through Duke University as an Integrative Health and Wellness Coach and through the Institute for Functional Medicine. After a decade of practicing Midwifery in the Quad Cities, she has transitioned the focus of her practice to wellness and weight management, acting as the Program Director for Weight Management at The Group.
Rachel’s integrative approach to optimal health encompasses nutrition, exercise, and stress management: all the components of lasting lifestyle change.
Taylor Streitmatter, Nutritionist
Taylor Streitmatter, The Group’s Nutritionist, earned her bachelor’s degree in Nutrition/Dietetics from Illinois State University in 2016. As a rural Sparland, Illinois native, she is excited to be helping patients close to home. As an integral part of the Weight Management Program team at The Group, she enjoys teaching patients about proper nutrition, particularly preventative nutrition education, to be the healthiest versions of themselves. Taylor’s favorite part about working at The Group is helping patients progress toward goals they never thought they’d be able to achieve.
When she’s not at The Group, Taylor loves cooking and baking, going to the gym, and playing guitar. She’s a St. Louis Cardinal fan and spends as much time as possible outside.

Allison Elfline, Personal Trainer
Allison Elfline, a Midwest native, has had many life experiences leading to her current role as trainer, leader, and coach. Allison attended Clarke College in Dubuque, Iowa, obtaining a degree in Musical Theatre, graduating in 1999 with honors. In 2002, she and her husband moved to Cincinnati, Ohio, where Allison enjoyed a career in semi- and professional theatre for many years. With a move home to the Quad City area, Allison focused her studies on obtaining a Master of Science in Education, with a focus on Clinical Mental Health Counseling and Behavioral Health. She graduated with honors in May 2014.
Over the years, Allison discovered a passion for combining her love of helping others with a love of nutrition, fitness, and general wellness. Allison also recently finished her education with the National Academy of Sports Medicine, becoming a Certified Personal Trainer, with specializations in Fitness Nutrition and Women’s Fitness.
