Insurance Claims
As a courtesy to you, our staff will submit insurance claims for your medical care, so long as the proper insurance information is available to us on your first visit. Please bring in your insurance ID card for your primary and secondary insurance to every visit.
You are responsible for any amount not paid by your insurance carrier. There are often co-payments, coinsurances, deductibles, and other charges not covered in full by insurance carriers. These amounts are due and payable at the time of each service or treatment. For your convenience, we accept cash, personal check, money order, Mastercard, Visa, or Discover for payment.
If you do not have insurance, payment is due in full at the time of service and before your delivery date. We will work with you to design a payment schedule that fits your needs. Please contact our Billing Office at (563) 355-1853 to make financial arrangements.

Billing for Prenatal Care
We understand that maternity benefits can be confusing. Our billing staff is available during normal office hours to discuss any questions you may have. Their phone number is (563) 355-1853. You will also have a visit with our billing staff to review your insurance coverage and your financial responsibility at your 1st OB appointment.
FMLA or Short Term Disability
We complete patient FMLA forms at no charge. All additional forms will have a $10 fee.
Payment must accompany form(s) prior to completion. Expect 7-10 business days for completion of forms.
Forgot Your Insurance Card? Effective May 16th, 2016
Our policy is that payment must be made at the time of service. Acceptable methods of payment are cash, check (with license number and expiration date) and credit or debit card.
Gynecology Visit Policy:
Our office staff will attempt to verify a patient's insurance eligibility by accessing the insurance company's website.
If insurance eligibility can't be verified, or a patient is unable to provide their insurance identification card or proof of insurance, the patient must pay the office fee at time of service.
Our office charges a minimum fee of $50.00 if you have forgot your insurance card (this is not the total cost of the visit) or reschedule your appointment until you have your card. Any additional services such as ultrasounds, procedures, labs etc. are billed in addition to the office visit.
If the patient does bring the insurance cards in to us at a later time, we will make the appropriate adjustment.
OB Visit Policy:
Our office staff will attempt to verify a patient's insurance eligibility by accessing the insurance company's website.
Our policy is that if you do not have your insurance card or proof of insurance at your NOB Exam appointment you will pay a minimum office fee of $50.00 or reschedule your appointment.
If insurance eligibility can't be verified by the third OB visit, or a patient is unable to provide their insurance identification card, the patient must pay for the visit in full at the time of service and any remaining balance.
